HGlobal’s mHub Social Prescribing solution enables healthcare providers (community paramedics, nurses, primary care, allied health etc.) to refer patients to local, non-clinical services, including community and social services.
These “prescriptions” enable providers to enhance support of patients by connecting them to resources directed at addressing social risk factors such as housing, food insecurity and loneliness. Attending to social determinants of health, social prescribing aims to reduce barriers to health and wellbeing and help address gaps in population health.
Social and Community services are added through mHub’s Admin portal which serves as a platform for healthcare providers to initiate, track and monitor their social prescribing activities.
